I acknowledge the physical nature of basketball and agree to the liability waiver outlined in the Player Waiver form above. I understand that B.A.L.L. is not responsible for any injuries sustained during league activities.

Age Requirements
Each team must maintain:
- 5 players aged 40+
- 4 players aged 39 or younger
Age determined by season start date.
